All parents (listed on the birth certificate) are required to sign the registration. If there is ONE PARENT on the birth certificate please bring it to your appointment as the doctor must sight it. Failure to do so may result in your appointment being rescheduled.
If there are COURT OR DVO ORDERS in place please bring them to the appointment as the doctor medico-legally must review these records.

If your are under the care of a specialist, please arrange a referral letter outlining your medical history and documentation demonstrating it is safe for you to have a Tongue / Lip Tie.
If you are any blood thinning medication you will need to see your GP to see if you can safely stop taking any blood thinning medication.
